The Caregiver Support Coordinator (CSC) is a Senior Social Worker whose primary responsibilities are to provide clinical services, program development, community outreach, resource development, continuous quality improvement activities, and evaluation/consultation and patient/staff education on care giving issues. The CSC is administratively responsible for the clinical programming and daily operation of the program, as well as managing data related to the program.

The Caregiver Support Coordinator develops and implements the Caregiver Support program specific to his/her area of assignment and insures compliance and collaboration with the greater NCHCS and the Veterans Administration Central Office caregiver support program initiatives.

The incumbent participates in direct patient/caregiver contacts to identify caregiver stress, assess caregiver burden and the development and application of directed clinical treatment interventions.

The incumbent conducts individual and family clinical interventions to facilitate the identification of caregiver stress, psychiatric illness, emotional distress and social needs as treatment issues. Professional judgement, including the knowledge of normal and abnormal behavior, is an inherent competency applied in daily interactions with veterans and their caregivers.

The incumbent provides referrals to VA medical and mental health treatment, and other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. If appropriate, the incumbent provides referrals or consults, or in some cases direct admission of the veterans to other VA programs including the Community Residential Care Program, Respite Program, Community Health Care Coordination Program or Community Nursing Home Program.

The incumbent serves as the major consultant for information on negotiating the complex processes the veteran and family may encounter when interacting with government and community agencies.

As a Senior Social Worker, the incumbent stays abreast of current federal and state legislation directed at assisting caregivers to continue to provide caregiving in an environment that is safe for both themselves and the patient. The incumbent will maintain a knowledge base of current theory and treatment modalities by reading professional journals, attending conferences, clinic seminars and teleconferences, and materials distributed through VA and private sector sources.

The CSC will collaborate with the patient care medical home medicine and geriatric primary care clinicians to provide patient and staff education on caregiver issues. Coordination will include hands-on support group work involvement, and training of patient home care clinical social work staff in the development and facilitation of such groups.

The CSC will develop a library of caregiver support information material for distribution through the various inpatient/outpatient and specialty clinic settings. Educational material must be specific to the age group serviced in the clinic, i.e. Alzheimer's disease and Parkinson disease in the general and geriatric settings and PTSD and TBI information in the general, OEF/OIF, post deployment and Polytrauma Center settings.

The Caregiver Support Coordinator will develop and update a NCHCS Caregiver educational resource library which is available both on-line and SharePoint accessible, pertinent to both clinicians and caregivers alike.

The CSC in collaboration with the OEF/OIF Program, Polytrauma Center and post deployment clinical staff will establish a caregiver support protocol (resources, education and training) sensitive to the issues of the caregivers who are providing care to all returning veterans.

DRIVER'S LICENSE: A current, valid driver's license is highly recommended for this vacancy.
