Responsibilities

This position is located at the Fort Collins, Colorado Outpatient Clinic for the Cheyenne VA Medical Center in Cheyenne, Wyoming and functions as the Housing and Urban Development-VA Supportive Housing (HUD-VASH) Case Manager (Social Worker) within the Mental Health Service. Duties may include, but are not limited to:

-Responsible for providing case management services to Veterans eligible and approved for the HUD-VASH housing voucher; providing case management services (directly or through referral) necessary to ensure that Veteran care is coordinated with the Veterans Affairs Health Care System (VAHCS), community health or mental health providers, and agencies that serve homeless populations; and serving as the primary contact with local Public Housing Agencies (PHAs).

-Works closely with the VAHCS's homeless programs, residential and community care programs, and other community partners that provide services to Veterans experiencing homelessness to ensure that coordination between these entities is facilitated and Veteran direct care, referral, and follow up is contiguous and based on a continuum of services.

-Independently provides clinical psychosocial and case management services that are at a professional, independent practice level for eligible Veterans and their family members in the program.

-Continuous coordination with the community's homeless services providers and coalitions, as well as with other local, state, and federal agencies.

-Provides crisis intervention when indicated including evaluation, assessment, triage, initiation of/support in Emergency Detention procedures within scope of licensure, and disposition of patients experiencing behavioral health emergencies.

-Attends Mental Health staff meetings and any other mandatory meetings regularly.

Work Schedule: Monday - Friday, 8am - 4:30pm
