Assess client health problems and needs, develop and implement nursing care plans, and maintain medical records for C1st and CMSPrograms in Bryan, Chatham, and Libety Counties.Incumbent will be required to make contact with children and families with or at risk of developing medical and/or developmental impairments. Incumbent may advise clients on health care, health maintenance and/or disease prevention and provide case management. Incumbent will makes hospital and home visits to assess the needs of client and the client's eligibility for other programs.

Job Responsibilities

Job Responsibilities & Performance Standards:

1. Performs job responsibilities with minimal supervision
2. Provides home visitation to at-risk infants and children
3. Serves as job expert or organization resource in the C1st/CMS programs
4. Consults and coordinates with health care team members to assess, plan, implement and evaluate client care plans
5. Maintains accurate, detailed reports and client records
6. Modifies client treatment plans as indicated by clients' responses and conditions
7. Assess client's health care needs, may provide referrals to other programs for care or assistance, and provides ongoing management of case load

Minimum Qualifications

Bachelor's degree in nursing from an accredited college or university AND One year of PHN experience AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate's degree in nursing from an accredited college or university AND Two years of PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.

Preferred Qualifications: BSN, Previous home care and/or medical case management experience preferred. Newborn and/or Pediatric experience preferred.

Applicants hired for the position with an ASN degree will berequired to successfully complete an approved School of Nursing BSN Level Health Assessment Course if they have not previously done so. The course fee is paid for by the Coastal Health District however if the course is not successfully completed or the person is separated from employment (voluntarily or involuntarily) they will be responsible for reimbursing the Coastal Health District for the cost of the course.
